Excel Based Application Form
Download the Excel Spreadsheet Template File "TrainingApplication2003P1_.xlt" by saving the file to your local computer. If you are using Netscape, a right click on the link may be necessary to bring up a pop-up menu with the "Save Link As" menu item.
Do Not attempt to fill-out the form over the internet or with a web browser like Netscape or Internet Explorer. You must (1) download the spreadsheet and (2) open it locally with Excel. Then (3) fill-out the form, and (4) save the spreadsheet under a new file name. Finally (5) the completed form can be e-mailed to WCT@LSU.edu as an attachment.

The following instructions are provide in the spreadsheet form:
The Excel Spreadsheet Template named "TrainingApplication2003P1_.xlt" contains macros which can be considered a security risk in some computing environments. If macros are "Disabled," the applications sheets can be printed and completed by hand for mailing or faxing.An Adobe "Portable Document Format" file named "TrainingApplication2003_P1.pdf" should have also been included with the application packet. The .pdf file is a printable version of this Excel Spreadsheet Template and can be used instead of "TrainingApplication2003P1_.xlt."
In order to complete and submit this application electronically, the security setting for Excel must be set to "Medium" or "Low."
With Excel running, select "Tools" >> "Macro" >> "Security" and select either the "Medium" or "Low" setting. Click the "OK" button. Then completely close Excel. The next time you run Excel, the new security setting will be in effect.
When you open "TrainingApplication2003_P1.xlt" with a medium security setting, Excel will display a prompt asking to "Enable" macros for the workbook. Click the "Enable" button if you want to fill-out the forms electronically.
After you complete the form, save the workbook with a new file name (preferably the applicants first and last names).
To submit the completed application electronically, e-mail the file as an attachment to: WCT@LSU.edu
Put the applicant's First and Last names in the subject line. Include a contact phone number and reply to e-mail address in the body of the e-mail.
